Acute coagulopathy is present on admission to the ED and is independently associated with significantly higher morbidity and mortality. Although strongly associated with shock and increasing injury severity, the mechanism through which this entity develops is unclear. Children presenting to the ED with an established coagulopathy must be recognized as early as possible and managed directly and aggressively. Exactly how to achieve this will be the subject of further clinical research over the next years.
