Prognosis of Patients With Heart Failure and Preserved EF
Smith and colleagues addressed the problem of determining the prognosis of heart failure patients with preserved ejection fraction (EF) and published their results in the Journal of the American College of Cardiology. They followed 413 patients with heart failure for 6 months to determine whether patients with EF ≥ 40% fared clinically better or worse compared with patients who had depressed EF.

Smith and colleagues discovered that patients with heart failure and preserved EF had a 6-month mortality rate of 13%, whereas patients with depressed EF had a 6-month mortality rate of 21%. Otherwise, the groups were similar with regard to the rate of functional decline and the readmission rate.

The authors conclude that patients with heart failure whose EF is preserved have a graver prognosis than has been previously stated. They also call for a better therapeutic strategy to manage these patients.
