Requesting a Report in Person

• 1). Pick up a death record application form in the lobby of the Ramsey County Department of Public Health at 555 Cedar Street in St. Paul. You can also download a form online.
  
  
• 2). Determine whether you have a tangible interest.
  
  
• 3). Complete the application, and sign it in the presence of a witness in the Office of Vital Records.
  
  
• 4). Provide personal identification.
  
  
• 5). Pay the required fee, $13 for the first copy and $6 for each additional copy.
  
  

Requesting a Report by Mail

• 1). Download and print the Ramsey County Death Record Application form from the Ramsey County website by clicking on the Death Records Application tab on the site's homepage.
  
  
• 2). Determine whether you have a tangible interest.
  
  
• 3). Complete the application, and sign it in the presence of a Notary Public.
  
  
• 4). Enclose a photocopy of your driver's license or identification card, and a check or money order for $13 payable to Ramsey County.
  
  
• 5). Mail to:
