Changing Your Name with the Florida Bar

• 1). Write a letter to the Florida Supreme Court Clerk's office requesting that your name be changed on the official roll of attorneys with the court. This letter must include your old name, your new name and your Florida Bar number. The letter should be sent to the Florida Supreme Court, with attention to the Clerk's Office at 500 South Duval Street, Tallahassee, Florida 32399-1927.
  
  
• 2). Include a self-addressed stamped envelope with your request to change your name, so that the Florida Supreme Court may send you a copy of the order approving your name change.
  
  
• 3). Send a copy of the Florida Supreme Court's Order changing your name to the Executive Director of the Florida Bar since members are required to notify the Florida Bar of any changes in their name or contact information. This letter should be sent to: The Florida Bar, with attention to the Executive Director at 651 E. Jefferson Street, Tallahassee, Florida 32399-2300.
